Customization – Choose multiple colors?

Let’s say I want to sell a customized item where the design is 3 stripes and then some custom text.

Is it possible to set up the customization on a product so that shoppers can choose Stripe Color 1, then Stripe Color 2, then Stripe Color 3, then enter their Custom Text?

I can’t seem to figure out a way to do this, and I’m wondering if it is not possible with Amazon Seller’s customization options.
