Product Inserts for Emails / Warranty

I am trying to get customer data for my customer sales on amazon and one way Im aware of is through a QR code/warranty. I am confused how to set this up. Does the QR code cake them to a website? What information would you typically ask for? Is there a solution/app/site that sets all of this up?
