Amazon FBA Commercial Invoice (UK to USA)

We are almost ready to send an FBA shipment to amazon USA, we have an EIN number but need to see a dummy invoice that is sent with the goods, as we have no idea what details are required when it comes to sending them.

Any help with this would be greatly appreciated.
