Need help applying for resellers permit

Hey guys I just started my llc in indiana and I am trying to find out which form I need to fill out for my resellers permit. I’ve tried looking it up but the closest thing I’ve found for it is the ST-105 form. However everytime I’ve found a link to the form it’s literally just the form and instructions on how to fill it out.

The problem is it doesn’t say if I need to fill it out and send it to the DOR or if I just fill it out and hand it to whichever wholesaler I’m buying stuff from.

Has anyone here ever gotten their resellers permit in indiana that could help me out?
